Student Accessibility and Inclusive Learning Services Grievance Process

Students who have a concern related to the provision of accommodations and/or services provided through the Student Accessibility and Inclusive Learning Services Office may submit their appeal/grievance with details through the following form. Please note that forms submitted by parents/guardians or other parties which are not the student will not be reviewed. 

Once your form is submitted, the appropriate supervisor will review the information as soon as possible. They will follow up with you regarding your submission and may include some preliminary questions to gain further insight into your concerns. Students do not have to provide additional insights at that time; however, it is recommended as decisions can only be made based on the information provided. Once the preliminary review and follow-up is complete, the information will be sent to a committee for formal review. 


The appeal/grievance review committee will evaluate the totality of information and follow up with the student directly via their ASU email.
