As part of our Community of Care, we ask that you receive a COVID-19 test before coming to campus in January.
Secure your test results no later than January 7.

Submitting Your Results

Please upload documentation of your test to the ASU Health Services portal at myhealth.asu.edu. You may be asked for evidence of completion. If you tested positive you are expected to remain isolated until your period of isolation is complete according to CDC guidance (https://www.cdc.gov/coronavirus/2019-ncov/your-health/quarantine-isolation.html).

 

If you did not test for COVID-19 between January 1 and January 7, 2022, please select from the options below.

 

Please keep documentation of your exemption or previous positive test in the event that you are asked for evidence. If you do not have an exemption for testing, you will be asked to test within the first week of your return to campus.
