SRVP Nomination form

Thank you for taking the time to nominate a fellow Sun Devil to become involved in sexual and relationship violence prevention initiatives at ASU.  This nomination form can be used to nominate ASU students, faculty, staff, departments, student groups and coalitions.

Following your submission, a Sexual and Relationship Violence Prevention team member will reach out to the nominated individual or group to discuss their involvement and to invite them to learn more. Some of the information included in this form may be provided to them, though you can choose to remain anonymous.

Please note, the programs found within this form are connected to the prevention of sexual and relationship violence, a topic of a highly sensitive nature. Engagement in violence prevention is unique to each individual and we encourage everyone to engage in the level that is supportive of their health and well-being. In light of the sensitive nature of this topic, it may be advisable to speak with the person or group you are thinking of nominating prior to doing so.
